Support Services

  • We provide Case Coordination & Case Management services for clients.  This is an additional support we provide in order to meet clients specific needs to assist them in getting help.  Upon intake, clients are assigned a Case Coordinator or Case Manager.  These team members ensure our Medicaid clients’ needs are being met emotionally, mentally, and physically. In addition, our team members are in charge of completing Service Plans. Lastly, these team members provide monitoring, linkage, advocacy, referrals, and crisis intervention.
  • Independent Rehabilitation Services (Social Skills) assists clients in developing and restoring basic life skills necessary to reduce or eliminate specific behaviors so they can function independently in their home or out in the community.
  • Peer Support services complement clinical services by providing support to clients in achieving and maintaining personal or social goals, where a greater wellbeing is sustained, all with a shared understanding of recovery.
  • Wraparound Services is an individualized approach to helping children, youth, and families with complex needs. The Wraparound process brings people together from different areas of a family’s life to form a team that helps youth stay in their homes, schools, and communities.

Medication Management

We offer medication management through tele-health services in partnership with Dr. Alex Padilla

  • Dr. R. Alexander Padilla is a nurse practitioner from the New York tri-state area and has almost 30 years’ experience in the healthcare industry. He is a Doctor of Nursing Practice specializing in Mental Health, and has spent over 15 years in various medical specialties such as oncology, autoimmune, hospice, ER, ICU, CCU, infectious disease and Hi-Tech Infusion Therapy.
